By checking this box I understand that all payments made are non refundable. Designs by Karinda LLC will not be responsible for students that do not show up on picture day at their assigned time. A make-up date will not be available, if you miss pictures day you are forfeiting all monies paid. If for any reason picture day is postponed due to Government Regulations picture day must be canceled on the set date, we attempt to schedule a make up date later in the Fall, if for some reason this is not an option, monies paid will be refunded except for a 5% credit card fee. *

Would you like any sibling pictures taken? Or pictures of your child with a buddy? If so this picture will be taken at designated picture time of the youngest sibling/buddy. Please have all siblings or buddies dressed and ready to go. Siblings must be enrolled in Dance at West Coast Dance Conservatory. *only submit this information on the class time that the buddy or sibling picture will take place* You will be able to choose which package or products you would like for each buddy or sibling picture on the next page.
